Depending on your insurance company, you may require a referral for services outside our office. This is especially true for managed care plans. To make the referral process as smooth as possible, we have adopted the following policies:

  • In order to avoid delays, we require as much notice as possible for your referral forms (minimum of 24 hours)
  • Your referral will be ready in our office for you to pick up.
  • We do not back-date referrals. Insurance companies do not allow us to give any referrals for services that have already been rendered.
